Walk-through queries
Keep the purpose modest and useful: notice conditions, arrange appropriate action and check what changed.
Does completing a checklist prove compliance?
No. A checklist records only the observations made within its scope. It cannot replace applicable requirements, competent assessment, required inspections or testing.
How often should a walk-through happen?
Base the schedule on hazards, change, incident history and applicable requirements. Add focused reviews after relevant changes or reports. Do not substitute this schedule for mandatory inspection intervals.
What should I do about an immediate danger?
Follow your workplace emergency, stop-work, isolation and escalation arrangements as appropriate to your role. Do not delay protective action to finish a record or use this website as an emergency channel.
What belongs in a useful observation?
Record the location, task, condition and people or activities exposed. Separate observed facts from assumptions. Include the next action, responsible role, review point and any interim protection.
Who should own the action?
Choose a role with the authority and resources to arrange the change. If several teams are involved, name a coordinating owner and clarify the supporting responsibilities.
When can an item be closed?
Close it when the agreed change is implemented and its effectiveness is verified in representative use. Record evidence and remaining limitations. Reopen the item if the problem returns.
What if I cannot check an area or understand a hazard?
Record it as not checked or requiring specialist review, with the reason and a responsible follow-up role. Do not guess, enter restricted areas or treat missing information as a pass.
Should I take photographs or name workers?
Follow workplace privacy, security and reporting rules. Prefer descriptions of conditions and responsible roles. Avoid identifying people or capturing confidential information unless properly authorized and necessary.
Does the observation composer submit my record?
No. It builds a plain-text record in your browser. You can copy or download it, then use your approved reporting process. The composer does not submit, upload or save the text to this website.
Requirements vary by location and activity. Refer technical or legal questions to a suitably qualified person and the relevant authority.
